Admissions Process

Requirements Prior to Admission:
1. Submit an application for admission with the $75.00 non-refundable application fee. 2. Complete a full-day student interview and visit. Please contact Erin Daunic, 615-354-1524, to arrange an appointment. 3. During the visit, the applicant will shadow a current student and also interview with the CHS administration to discern appropriateness of academic and therapeutic fit. 4. Submit a release for information from prior treatment centers, therapists, doctors and schools. 5. If your student is admitted to CHS, our family will receive an acceptance letter packet. These forms need to be completed and submitted on your student's official enrollment date which will be provided in the acceptance letter.
